Our goals:

At the Tanner clinic our patients are the center of all that we do.  Our physicians and team members are committed to providing the highest quality of care during all stages of your treatment.  With this in mind, we want to share with you our process for completing disability and FMLA forms​.

Short-term Disability:

  1. If a discussion regarding necessity of FMLA or short term disability is needed a separate dedicated office visit is requested so that a thorough evaluation of need can be determined. 
  2. Once need is determined by your physician and the patient obtains the appropriate forms from their employer/insurance/other agency, a nurse's visit must be made in order to complete each form based on the predetermined discussion of your physician.  (In the unlikely event this is not a routinely covered service under your insurance you will be directly responsible for this visit charge.  Standard office charges,   terms,  and policies will apply).  Please do not "walk in", or drop off forms or letters as your physician (if in the office) is currently providing care to other scheduled patients and this can result in lost or damaged paperwork, and therefore we cannot accept responsibility.
  3. Patient information portion of the form must be completed prior to processing.
  4. Once we have reviewed your form(s) and your signed authorization to release your medical information, please allow 5 – 7 business days for the processing of the form.  All forms will be faxed to the disability carrier/employer as indicated by the patient, or may be picked up at our office.  It is your responsibility to obtain a copy for your personal records at this visit if your employer/disability carrier is unable to receive this by fax.​
  5. A copy of the form will be maintained within your permanent personal record.  Any further copies of these records will incur a standard charge for duplication of medical records.  If needed this can be obtained ONLY from our medical records department located at the Layton Tanner clinic (Tanner Clinic.  2121 N 1700 W Layton Utah. (801) 773-4840).  Do not contact the physician directly as they will be unable to assist you.  If you are not located within the immediate area and are unable to obtain your records physically our patient portal is also an excellent way of being able to electronically obtain your medical records.  Please contact our medical records department or the patient account specialist who would be happy to further assist you.

Long Term Disability

Our physicians are extremely good at what they do unfortunately medicine is complex as is disability complications.  Because of this a specialist referral is needed to personnel with specific training in Physical Medicine and Rehabilitation (aka PM&R or Physiatry), and/or Occupational (workplace) Medicine to best help your immediate and long term disability concerns. 
These physicians/practices are dedicated to helping individuals that may be in need of long-term disability assistance.  We have provided for your benefit a list of some of those physicians certified in assisting in these cases.  In the case that your insurance requires a referral letter please verify from whom this referral letter is needed (your original treating physician, primary care provider, surgeon etc.) It is the patient's responsibility to contact their insurance for a specific list of those providers to whom coverage may apply.
 
Some helpful information in expediting this process would include:
  1. Call your insurance for a covered provider close to you. 
  2. Obtain all applicable medical records from your various providers and take them at your 1st appointment.  Your disability specialist will likely not be able to process all this information at that time but it will allow them to obtain background information and organize your case reducing the need of additional further visits, cost, and valuable time. 
  3. Be careful in "doctor shopping" as this is often seen in a negative light regarding your final determination of your carrier. 
  4. Be understanding in the fact that this clinic often needs time in order to compile an appropriate patient profile allowing you to present your case as needed.
  5. Until your care is complete with your original doctor's  office continue your course of care/recommendations as failure to do so may result in decreased outcomes.
